Job summary

Qualifications

• CT RN license required.
• ASN degree required; BSN preferred.
• Two years of acute-care nursing experience; case management, home care, or managed care experience strongly considered.
• Excellent communication, negotiation and conflict resolution skills.
• Knowledge of computer applications and reimbursement systems preferred.
• Certification in case management desired.

Responsibilities

• Assess patients for appropriate level of care and coordinate their care across the care continuum.
• Facilitate involvement of support services and transition planning activities.
• Document care in case management systems and participate in quality improvement initiatives.
