Job summary

Hartford Hospital is seeking a Case Coordinator to enhance patient throughput by assessing appropriate levels of care and coordinating services across the continuum. The role centers on discharge planning and active patient advocacy within a nursing team.

Required are an active CT RN license and 2+ years of acute-care experience; ASN is required, BSN preferred. Strong communication, negotiation, and conflict resolution skills are essential for success in this role.

Qualifications

• CT RN license required.
• ASN is required, BSN preferred.
• Two years of acute-care and diversified clinical nursing experience; case management, home care, and/or managed care experience strongly considered.
• Excellent communication, negotiation and conflict resolution skills are required.

Responsibilities

• Facilitate patient throughput and assess patients for appropriate level of care.
• Coordinate patient care and services across the care continuum.
• Identify risk factors and involve support services as needed.
• Perform utilization management functions and develop transition planning activities.
• Document and utilize case management applications; collaborate with the health care team on process and quality improvement.
