Job summary

Hartford Hospital is seeking a Case Coordinator to support patient throughput in the Case Coordination Department. You will assess patients for the appropriate level of care, coordinate services across the continuum, and work with nurses and administration to ensure timely transitions.

The role requires an active CT RN license and at least ASN with 2 years of acute-care experience. The position involves rotating weekends/holidays and full-time hours in Hartford, CT.

Qualifications

• CT RN license required.
• ASN required, BSN preferred.
• Two years acute-care and diversified clinical nursing experience required; case management, home care, and/or managed care experience considered.

Responsibilities

• Assess patients for appropriate level of care and coordinate their care across the continuum.
• Identify risk factors and facilitate involvement of support services.
• Perform utilization management and develop transition planning activities.
• Document care in case management applications and collaborate with health care team.
