Job summary

The Healthcare Manager at UnitedHealth Group is responsible for supporting patients with high risk health conditions to navigate the healthcare system, acting as an educator, resource, and advocate to ensure a maximum quality of life. You will collaborate with physicians, nurses, pharmacists, social workers, and other educators in a less structured environment within the RN scope of practice.

Qualifications

• RN license and active/unrestricted license to practice in state of employment.
• BLS certification.
• 2+ years in physician’s office, clinical or hospital setting.

Responsibilities

• Identify high-risk patients with providers and clinic staff.
• Support longitudinal care for chronic conditions including assessment, medication reconciliation, motivational interviewing, patient education, and referrals.
• Coordinate transitions from inpatient to outpatient care and plan contingencies.
• Document accurately in the electronic medical record and participate in PCC meetings.
• Prepare reports for weekly meetings and maintain current clinical competence.
