Job summary

Responsibilities include identifying high-risk patients, coordinating transitions of care, and maintaining accurate documentation in the EMR. Requires RN licensure, BLS, and 4+ years in relevant settings.

Qualifications

• RN with RN license in the state of employment.
• BLS certification current.
• 4+ years in a physician's office, clinical or hospital setting.
• Knowledge of COPD/asthma, diabetes, CHF and IHD.
• Excellent verbal and written skills; strong interpersonal abilities.

Responsibilities

• Identify high-risk patients with providers and clinic staff.
• Support longitudinal care with assessment, reconciliation, education and referrals.
• Coordinate transitions from inpatient to outpatient care with contingency planning and education.
• Document in the EMR accurately and timely; attend PCC meetings.
• Prepare reports for weekly leadership reviews; maintain nursing practice competence.
