Job summary

WellMed, part of the Optum family, seeks a Healthcare Manager to support high-risk patients and coordinate care across multidisciplinary teams. You will educate, advocate, and empower patients and families to improve quality of life while operating within RN scope.

The role emphasizes medication reconciliation, patient education, and transitional care, with collaboration among physicians, nurses, pharmacists, social workers, and other educators. Two languages are valued.

Qualifications

• Bachelor's or Associate degree in Nursing with RN license.
• Current BLS certification.
• 4+ years RN experience in clinical/hospital/physician setting.
• Active RN license in state of employment.
• Experience with patient education and care coordination.

Responsibilities

• Identify high-risk patients and coordinate longitudinal care.
• Perform medication reconciliation and patient education.
• Support inpatient-to-outpatient transition planning.
• Document accurately in the EMR.
• Participate in PCC meetings and daily huddles.
• Develop referrals to community resources.
• Maintain up-to-date knowledge of evidence-based practices.
