Job summary

UnitedHealth Group, through WellMed and the Optum family, seeks an Healthcare Manager to support high-risk patients, acting as educator, resource, and advocate to optimize quality of life. You will work with multidisciplinary teams and perform nursing duties under state RN scope.

Qualifications

• Bachelor's degree in Nursing or Associate's degree in Nursing with 4+ years of RN experience
• Active RN license to practice in the state of employment
• Current BLS certification
• 2+ years of experience in a physician's office, clinical or hospital setting
• Knowledge of chronic diseases (COPD/asthma, diabetes, CHF, IHD)
• Proficient EMR skills
• Strong verbal and written communication
• Organizational and prioritization skills
• Interpersonal and team collaboration skills
• Tuberculosis screening and immunity proof required

Responsibilities

• Identify patients at high risk with providers and clinic staff
• Support longitudinal care: assess health conditions, med reconciliation, motivational interviewing, patient education, and referrals
• Assist with inpatient-to-outpatient transition: assess transitional needs, med reconciliation, contingency planning, education, post-discharge support
• Coordinate with providers to update the plan of care
• Document accurately in the electronic medical record
• Participate in daily huddles and PCC meetings
• Prepare reports for weekly meetings
• Maintain competence in nursing practice and evidence-based care
• Perform other duties as assigned
