Job summary

UnitedHealth Group in Texas is seeking a Healthcare Manager to support patients with high-risk conditions and help them navigate the care system. The role emphasizes patient education, advocacy, and collaboration with physicians, nurses, pharmacists, and other care team members to improve quality of life.

You will document in the electronic medical record, participate in PCC meetings, and help develop and update plans of care with a focus on transitioning patients from inpatient to outpatient

Qualifications

• RN with active license in the state of employment.
• Current BLS certification.
• 2+ years of experience in a physician's office, clinical or hospital setting.
• Knowledge of chronic diseases (COPD/asthma, diabetes, CHF, IHD).
• Proficient computer skills with electronic medical records.

Responsibilities

• Identify high-risk patients with providers and clinic staff.
• Support longitudinal care, including assessments and education.
• Coordinate care transitions from inpatient to outpatient settings.
• Document accurately in the EMR and participate in PCC meetings.
• Assist with post-discharge needs and referrals.
