Job summary

The position requires an active RN license, BLS, and 2+ years in clinical or office settings, with proficiency in EMR and strong communication skills. Fluency in English and Spanish is a plus; on-site work is required in many clinics.

Qualifications

RN with active, unrestricted license in state of employment.
Current BLS certification.
2+ years of experience in physician's office, clinical, or hospital setting.
Knowledge of COPD/asthma, diabetes, CHF and IHD.
Proficient with electronic medical records.
Excellent verbal and written communication.
Strong organizational and prioritization abilities.
Effective interpersonal skills and teamwork with multidisciplinary groups.
TB screening and immunity verification requirements.
Spanish language skills preferred.

Responsibilities

Support high-risk patients to navigate the healthcare system.
Educate, empower, and advocate for patients and families.
Collaborate with multidisciplinary teams including physicians, nurses, pharmacists, and social workers.
Document accurately in the electronic medical record.
Participate in PCC meetings and daily huddles.
Prepare reports for weekly meetings and maintain current nursing competencies.
May perform clinical tasks within license and scope.
