Job summary

Qualifications

• Bachelor's degree in Nursing or Associate's degree in Nursing with four+ years of RN experience
• Active, unrestricted RN license in the state of employment
• Current BLS certification
• 2+ years of experience in a physician's office, clinical or hospital setting
• Knowledge of chronic diseases (COPD/asthma, diabetes, CHF, IHD)
• Proficient with electronic medical records
• Strong verbal and written communication
• Excellent interpersonal and organizational skills
• TB screening and immunity proof for MMR, Varicella, etc.

Responsibilities

• Identify high-risk patients with providers and clinic staff
• Support longitudinal care, assess health conditions and med reconciliation
• Assist in transitions from inpatient to outpatient settings and education
• Coordinate with providers to update care plans
• Document accurately in the EMR and support PCC meetings
• Prepare reports for weekly meetings and maintain best practices
