Qualifications

• RN license with active, unrestricted practice in the state of employment.
• Current BLS certification.
• 4+ years in a physician's office, clinical, or hospital setting.
• Knowledge of chronic diseases (COPD/diabetes/CHF/IHD).

Responsibilities

• Identify high-risk patients with providers and clinic staff.
• Support longitudinal care, including assessments and education.
• Coordinate care transitions from inpatient to outpatient.
• Document accurately in the EMR and participate in PCC meetings.
