Job summary

WellMed, part of the Optum family, is seeking a Healthcare Manager to support high-risk patients in navigating the healthcare system. The role works with multidisciplinary teams to educate, advocate, and empower patients and families while performing nursing duties within the RN scope.

The position emphasizes care coordination, transitions from inpatient to outpatient care, and timely documentation. BLS is required, with a focus on improving health outcomes in a dynamic environment.

Qualifications

• RN with current, unrestricted nursing license in the state of employment.
• Bachelor's or Associate degree in Nursing.
• Current BLS certification and at least 2+ years in a clinical setting.

Responsibilities

• Identify high-risk patients with providers and clinic staff.
• Support chronic care management, including assessments and patient education.
• Coordinate transitions from inpatient to outpatient care and manage contingency plans.
• Document accurately in the EMR and participate in PCC meetings.
• Prepare reports for weekly meetings and support ongoing nursing competence.
