Job summary

WellMed, part of the Optum family of businesses, seeks a Healthcare Manager to support high-risk patients and coordinate care across multidisciplinary teams. The role emphasizes education, advocacy, and transition planning.

The RN will work in a less-structured environment, perform nursing duties under the state scope, document accurately in EMR, and contribute to weekly PCC meetings while promoting evidence-based practice.

Qualifications

• RN licensed and in good standing in the state of employment
• Current BLS certification
• 2+ years' experience in a physician's office, clinical, or hospital setting
• Knowledge of chronic diseases (COPD/asthma, diabetes, CHF, IHD)
• Proficient with electronic medical records (EMR) and documentation
• Strong verbal and written communication; organized and collaborative

Responsibilities

• Identify high-risk patients with providers and clinic staff.
• Support longitudinal care: assess health conditions, med reconciliation, motivational interviewing, patient education, and referrals.
• Assist with transition from inpatient to outpatient care, with assessments, medication reconciliation, contingency planning, and post-discharge support.
• Coordinate with providers to establish or update the plan of care.
• Accurate and timely EMR documentation.
• Participate in daily huddles and PCC meetings; prepare weekly reports.
• Maintain current knowledge of evidence-based practices; perform other duties as assigned.
