Qualifications

Responsibilities

• Provides continuity of care by ensuring smooth transitions between care settings.
• Develops relationships with patients and the multidisciplinary team to navigate treatment and follow-up.
• Manages high-risk, complex patient care to minimize readmission.
• Collaborates with physicians to coordinate patient care plans and communications.
• Prepares and reinforces post-discharge care plans.
• Identifies barriers to care and supports changes in care processes.
• Adapts communication to diverse patient populations and respects privacy.
