Qualifikationen

• Develops relationships with patients and multidisciplinary team to facilitate follow-up care.
• Manages high-risk, complex patient care with the goal of minimizing readmission.
• Coordinates care plans with physicians and team; communicates with care team as the patient advocate.
• Prepares and reinforces post-discharge care plans.
• Identifies barriers to care and works to improve processes for patient navigation.
