The role requires an RN licensed in the state, 2 years nursing experience, BLS certification, and the ability to work a flexible schedule, travel between facilities, and advocate for patients throughout the treatment process.

Qualifications

• Graduate of an accredited school of nursing.
• Associate’s or Bachelor’s degree in nursing preferred.
• Two years nursing experience required.
• Current RN license in state of practice.
• Basic Life Support (BLS) certification.

Responsibilities

• Provides continuity of care across settings.
• Develops relationships with patients and multidisciplinary teams.
• Manages high‑risk, complex patient care to minimize readmission.
• Coordinates care plan communication with physicians and team.
• Prepares and reinforces post‑discharge care plan.
• Identifies barriers to care and suggests process changes.
• Assists leadership to improve patient experiences.
• Adapts communication to patient population and respects privacy.
• Performs other related duties as required.
