Job summary

Ochsner Health is seeking a registered nurse to serve as a liaison among patients, caregivers, providers, and post-acute care teams. You will facilitate discharge planning, coordinate transitions of care, and support ongoing treatment plans in collaboration with a multidisciplinary team.

The role requires an active RN license, BLS, and 2 years of nursing experience, with a preference for BSN or clinical specialty certification.

Qualifications

Responsibilities

• Provides continuity of care by ensuring smooth transitions between care settings.
• Develops relationships with patients and the multidisciplinary team to facilitate subsequent treatment and follow-up.
• Manages high risk, complex patient care to minimize readmission.
• Coordinates care plans with physicians and multidisciplinary team; serves as patient advocate.
