Job summary

Ochsner Health seeks a dedicated RN Care Coordinator to ensure smooth transitions across care settings and to support patients and caregivers throughout the treatment continuum.

You will collaborate with physicians and a multidisciplinary team to implement up-to-date, patient-centered care plans and to optimize transitions after discharge. Strong communication skills and a focus on reducing readmissions are essential.

Qualifications

• Graduate of an accredited school of nursing.
• Preferred: Associate's or Bachelor's degree in nursing.
• Two years of nursing experience.
• Current RN license in state of practice and BLS certification.
• Certification in clinical specialty area preferred.

Responsibilities

• Provides continuity of care by ensuring smooth transitions between care settings.
• Develops relationships with patients and multidisciplinary team to coordinate care.
• Manages high-risk, complex patient care to minimize readmission.
• Coordinates care plan communication with physicians and team.
• Prepares and reinforces post-discharge care plans.
• Identifies barriers to care to improve processes for patient navigation.
