Job summary

Ochsner Health in Louisiana seeks an experienced Registered Nurse to serve as a liaison across patient care settings, caregivers, and the care team. You will facilitate discharge planning, coordinate with physicians and the care team, and support transitions along the continuum of care.

The role emphasizes managing high-risk patients, optimizing outcomes, and ensuring clear communication to patients and families. Eligibility includes an active RN license and BLS certification.

Qualifications

• RN license in state of practice is required.
• Graduate of an accredited school of nursing is required; an Associate’s or Bachelor’s degree is preferred.
• A current BLS certification is required; certification in a clinical specialty is preferred.

Responsibilities

• Provides continuity of care by ensuring smooth transitions between care settings.
• Develops relationships with patients and the multidisciplinary team to support continuity of care.
• Manages high-risk, complex patient care to minimize readmission.
• Coordinates care plan communication with physicians and the multidisciplinary team.
• Prepares and reinforces post-discharge care plans.
• Identifies barriers to care and suggests process improvements.
• Collaborates with leadership to improve the patient experience across referrals.
