Job summary

Qualifications

• Proficiency with computers, software, and web-based applications.

• Ability to work a flexible schedule, including weekends and holidays, with travel between facilities.

Responsibilities

• Provides continuity of care by ensuring smooth transitions between care settings.
• Develops relationships with patients and multidisciplinary team to facilitate and navigate subsequent treatment and follow-up.
• Manages high-risk, complex patient care with the goal of minimizing readmission.
• Coordinates care plans with physicians and team; advocates for patient.
• Prepares, executes, and reinforces post-discharge care plan.
• Identifies barriers to care and proposes process improvements.
• Collaborates with leadership to improve the clinical experience for referred patients.
• Adapts communication to the patient population, respecting privacy and explaining services clearly.
• Performs other related duties as required.
