Ochsner Health seeks a registered nurse to join our patient care teams across the continuum. You will coordinate transitions, collaborate with physicians and multidisciplinary teams, and advocate for patients to support continuity of care and reduce readmissions.

The role requires a state RN license, 2 years of nursing experience, BLS certification, and flexibility to work a varied schedule; preferred BSN/ASN and clinical specialty certification.

Qualifications

• RN license in state of practice.
• Minimum 2 years of nursing experience.
• Basic Life Support (BLS) certification.
• Preferred clinical specialty certification.

Responsabilités

• Provides continuity of care by ensuring smooth transitions between care settings.
• Develops relationships with patients and multidisciplinary team to facilitate subsequent treatment and follow-up.
• Manages high-risk, complex patient care to minimize readmission.
• Coordinates patient care plans with physicians and team; advocates for patient.
• Prepares and reinforces post-discharge care plan.
• Identifies barriers to care and helps improve care processes.
