Job summary

Ochsner Health is seeking a registered nurse to serve as a liaison among patients, caregivers and the multidisciplinary team. The role emphasizes continuity of care, discharge planning, and coordination across facilities to ensure quality, cost-effective outcomes.

The ideal candidate will have an RN license, 2 years of nursing experience, and BLS certification, with preference for a clinical specialty certification. Flexibility to travel between facilities is needed.

Qualifications

• Proficiency in using computers, software, and web-based applications.
• Effective verbal and written communication skills and ability to present information clearly.
• Excellent conflict resolution skills.
• Ability to work a flexible work schedule (e.g. 24/7, weekend, holiday, on call availability) and travel between facilities.

Responsibilities

• Provides continuity of care by ensuring smooth transitions between care settings.
• Develops relationships with patients and their multidisciplinary team to facilitate and/or navigate through subsequent treatment and follow-up.
• Manages high risk, complex patient care with the goal of minimizing readmission.
• Coordinates patient care plan communication with physicians and multidisciplinary team as a patient advocate.
• Prepares, executes, and reinforces post-discharge care plan.
• Identifies barriers to care and suggests process improvements for patient navigation.
