Ochsner LSU Health in Louisiana seeks a Registered Nurse to serve as a patient-care liaison and advocate across the care continuum. You will coordinate with patients, families, and a multi-disciplinary team to ensure safe transitions and effective discharge planning.

The role requires an active RN license in the state, BLS certification, and at least 2 years of nursing experience; certification in a clinical specialty is preferred. Flexibility to travel between facilities is expected.

Qualifications

• RN license in state of practice (Current).
• Basic Life Support (BLS) certification (AHA).
• Graduate of an accredited school of nursing.
• 2 years of nursing experience preferred.
• Certification in a clinical specialty preferred.

Responsabilités

• Provide continuity of care by ensuring smooth transitions between care settings.
• Collaborate with patients, caregivers, and multidisciplinary team to plan follow-up.
• Coordinate patient care plans with physicians and care teams.
• Assist with discharge planning and post-discharge care execution.
• Identify barriers to care and advocate for process improvements.
• Maintain compliance with laws, regulations, and privacy standards.
• Flexible scheduling including on-call coverage and travel between facilities.

Graduate of an accredited school of nursing

Associate's or Bachelor’s degree in nursing
