Job summary

The successful candidate will have an active RN license, BLS, and 2+ years of nursing experience, with strong communication skills and familiarity with electronic health records.

Qualifications

• Registered nurse licensed in the state of practice.
• Current BLS certification.
• Experience coordinating patient care and discharge planning.

Responsibilities

• Ensure smooth transitions between care settings for patients.
• Coordinate post-acute care and follow-up with multidisciplinary team.
