Job summary

Point32Health is seeking a Care Manager – Nursing Field (RN CM) in Canton, MA to ensure timely care management across the continuum, including transitions of care, care coordination, and complex case management in collaboration with providers.

The role requires a bachelor’s degree in nursing, an active RN license, and at least 5 years of relevant clinical experience. A national case management certification is desirable. A $7,500 sign-on bonus is offered along with comprehensive benefits.

Qualifications

• Bachelor’s degree in nursing required.
• RN license in state of residence.
• Minimum 5 years of relevant clinical experience.
• National certification in Case Management desirable.
• Experience in home care or case management preferred.

Responsibilities

• Perform telephonic outreach and face-to-face encounters using motivational interviewing.
• Assess member needs, health goals, and barriers to wellness.
• Develop and implement member care plans with members and providers.
• Document according to policy and regulatory standards.
• Provide targeted health education and proactive strategies for condition management.
• Coordinate safe transitions after hospital discharge.
• Collaborate with interdisciplinary care team to improve outcomes.
• Present high-risk members at interdisciplinary rounds.
• Pursue professional growth through self-directed learning.
