Job summary

Point32Health is seeking a Nurse Care Manager (RN CM) to ensure timely care management across transitions of care, coordination, and disease management for members in Massachusetts. The role emphasizes clinical expertise, collaboration with caregivers and providers, and empowering members for optimal wellness.

The RN CM will document activities, educate members, and participate in interdisciplinary rounds, contributing to a comprehensive care strategy and improved health outcomes.

Qualifications

• RN with current unrestricted license in state of residence.
• Bachelor's degree in nursing or equivalent experience.
• Minimum 5 years of relevant clinical experience.

Responsibilities

• Perform telephonic outreach and care enrollment using motivational interviewing.
• Assess member needs, goals, and barriers to wellness.
• Develop and revise member-specific care plans with providers.
• Document in applicable systems per policy and standards.
• Provide health education and proactive condition management strategies.
• Coordinate safe transitions to higher or alternative care levels.
• Collaborate with care team to improve member outcomes.
• Present high-risk members at interdisciplinary rounds as needed.
