Job summary

Point32Health is seeking a SCO Nurse Care Manager to ensure timely care management across the continuum, including transitions of care and care coordination. This role requires strong clinical knowledge and critical thinking for experienced nurses.

The position is remote, MA-based, full-time, with a salary range of $87,063–$130,594 per year. A BSN, RN license, and 5 years of relevant clinical experience are required; CM certification is desirable.

Qualifications

• Bachelor’s degree in Nursing is required (preferably).
• 5 years of relevant clinical experience required.
• Current unrestricted RN license in state of residence.
• National CM certification desirable.

Responsibilities

• Perform telephonic member outreach and face-to-face encounters.
• Conduct departmental assessments to identify member needs and health goals.
• Develop and implement member-specific plans of care.
• Complete documentation according to departmental policy and regulatory standards.
• Provide targeted health education and proactive strategies for condition management.
• Collaborate with members and the facility care team for safe transitions of care.
• Liaise with the interdisciplinary care team to improve member outcomes.
• Attend and present high-risk members at interdisciplinary rounds.
• Maintain professional growth through self-directed learning activities.
