Job summary

Point32Health is seeking a Care Manager – Nursing Field (RN CM) to ensure timely care management across the continuum, including transitions of care and disease management per department guidelines. The nurse care manager collaborates with members, caregivers, and providers to empower self-management and achieve optimal wellness.

Licensed RN with a bachelor’s degree and 5+ years of clinical experience is required; CM certification is desirable.

Qualifications

• Active RN license in state of residence; national CM certification desirable.
• Bachelor’s degree or relevant equivalent experience.
• 5+ years of clinical experience; home care or case management preferred.

Responsibilities

• Perform telephonic outreach and assess member needs to enroll in programs.
• Develop and revise member-specific care plans with providers.
• Document in applicable platforms per policy and standards.
• Support smooth transitions of care and post-discharge follow-up.
• Collaborate with interdisciplinary team to improve outcomes.
