Job summary

The role requires a Bachelor’s degree in nursing, 5 years’ clinical experience, and an unrestricted RN license, with national Case Management certification preferred. Responsibilities include outreach, assessments, care plans, documentation, and collaboration with interdisciplinary teams.

Qualifications

• Bachelor’s degree required, preferably in nursing.
• 5 years’ relevant clinical experience required.
• Current unrestricted RN license in state of residence.
• National certification in Case Management desirable.

Responsibilities

• Perform telephonic member outreach and/or face-to-face encounters.
• Conduct departmental assessments to identify member needs and health goals.
• Develop and implement member-specific plans of care.
• Complete documentation according to departmental policy and regulatory standards.
• Provide targeted health education and proactive strategies for condition management.
• Collaborate with the facility care team for safe transitions to the next level of care.
• Liaise with the interdisciplinary care team to improve member outcomes.
• Attend and present high-risk members at interdisciplinary rounds.
• Maintain professional growth through self-directed learning activities.
