Job summary

Point32Health is seeking a Care Manager – Nursing Field (RN CM) to ensure timely care management across the care continuum, including transitions of care, care coordination, and disease management. The RN CM will collaborate with members, caregivers, and providers to empower optimal wellness and self-management.

Requirements include RN licensure, a Bachelor’s degree in nursing, and at least 5 years of clinical experience.

Qualifications

• Active RN license in the state of residence.
• Bachelor's degree in nursing required.
• Minimum 5 years of relevant clinical experience.
• Home health or case management experience preferred.

Responsibilities

• Perform telephonic member outreach and in-person encounters to enroll in programs.
• Assess members to identify needs, goals, and barriers to wellness.
• Develop and revise member-specific care plans in collaboration with the care team.
• Document care plans and interventions in required systems.
• Coordinate transitions of care and ensure effective post-discharge follow-up.
