Point32Health is seeking an RN Care Manager to ensure timely care management across the continuum, including transitions of care, care coordination, and complex case management. Ideal for experienced nurses with strong clinical knowledge and critical thinking.

Highlights include a pay range of $84,143–$126,215 per year, remote work in Massachusetts, and a $7,500 sign-on bonus. You will perform outreach, develop care plans, and collaborate with an interdisciplinary team to improve member outcomes.

Qualifications

• Bachelor's degree required, preferred in nursing.
• 5 years' relevant clinical experience required.
• Current RN License in state of residence.

Responsibilities

• Perform telephonic member outreach and/or face-to-face encounters.
• Conduct departmental assessments to evaluate member needs and health goals.
• Develop and implement member-specific plans of care.
• Complete documentation according to departmental policy and regulatory standards.
• Provide targeted health education and proactive strategies for condition management.
• Collaborate with the care team to coordinate safe transitions to the next level of care.
• Liaise with the interdisciplinary care team to improve member outcomes.
• Attend and present high-risk members at interdisciplinary rounds.
• Maintain professional growth through self-directed learning activities.
