Job summary

Point32Health is seeking a Care Manager - Nursing Field (RN CM) to ensure timely care management across the care continuum, including transitions of care, care coordination, and disease management. The RN CM will collaborate with members, caregivers, and providers to achieve individualized care goals and empower self-management.

The role requires strong clinical knowledge, critical thinking, and the ability to facilitate care plans that optimize quality and outcomes.

Qualifications

• RN with current, unrestricted license in state of residence or able to obtain
• Minimum 5 years of relevant clinical experience
• Experience in home health or case management preferred
• Proficiency in a second language desirable

Responsibilities

• Perform telephonic member outreach and/or face-to-face encounters to facilitate program enrollment
• Assess members holistically to identify needs and wellness goals
• Develop and revise member-specific plan of care with stakeholders
• Document in electronic records per policy and standards
• Coordinate transitions of care and post-discharge follow-up to prevent readmission
• Collaborate with interdisciplinary team to improve member outcomes
