Job summary

Point32Health is seeking an RN Care Manager in Canton, MA to coordinate transitions of care and complex case management, delivering member-centered care across the continuum.

A BSN is preferred with at least 5 years of clinical experience; an unrestricted RN license is required, and national case management certification is desirable. Qualifications

• Bachelor’s degree in nursing required; RN license in state of residence.
• Minimum of 5 years of relevant clinical experience.
• National certification in Case Management desirable.

Responsibilities

• Perform telephonic member outreach and face-to-face encounters using motivational interviewing skills.
• Conduct departmental assessments to identify member needs, health goals, and barriers to wellness.
• Develop and implement member-specific plans of care in collaboration with members and providers.
• Complete documentation according to departmental policy and regulatory standards.
• Provide targeted health education and proactive strategies for condition management.
• Coordinate safe transitions to the next level of care post-hospital discharge.
• Collaborate with the interdisciplinary care team to improve member outcomes.
• Attend and present high-risk members at interdisciplinary rounds.
• Maintain professional growth through self-directed learning activities.
