Job summary

Point32Health is seeking a Care Manager – Nursing Field (RN CM) to ensure timely care management across the continuum, including transitions of care, care coordination, and complex case management. This remote MA role requires strong clinical knowledge and critical thinking.

The position offers a salary range of $87,113–$130,669 per year with full-time schedule, comprehensive benefits, and opportunities for professional development.

Qualifications

• Bachelor’s degree required; Bachelor’s degree in nursing preferred.
• 5 years of relevant clinical experience required.
• RN license with current unrestricted status in state of residence; national Case Management certification desirable.

Responsibilities

• Perform telephonic outreach and/or face-to-face encounters.
• Assess member needs and health goals through department assessments.
• Develop and implement member-specific care plans.
• Complete documentation per policy and regulatory standards.
• Provide targeted health education and proactive condition management strategies.
• Collaborate with facility care team for safe transitions to higher levels of care.
