Job summary

Point32Health is seeking a Care Manager – Nursing Field (RN CM) to ensure timely care management across the continuum, including transitions of care, care coordination, and disease management. The nurse care manager will work with members, caregivers, and providers to develop and implement plans to support wellness.

The role requires an active RN license, 5 years of clinical experience, and proficiency with Microsoft Office and EHR systems.

Qualifications

• Registered Nurse with current unrestricted license in state of residence.
• May be required to obtain other state licensure where Point32Health operates.
• National certification in Case Management desirable.
• Bachelor’s degree or relevant equivalent experience; BSN preferred.

Responsibilities

• Perform telephonic outreach and assess member needs to enroll in programs.
• Provide health education and proactive strategies for condition management.
• Coordinate safe transitions of care and post-discharge follow-up.
• Collaborate with interdisciplinary care team to improve outcomes.
