Job summary

Point32Health is seeking a Nurse Care Manager (RN CM) to ensure timely care management across the continuum, including transitions, coordination, and disease management. You will empower members with strong clinical knowledge and collaborate with caregivers and providers to meet individual goals.

The role requires an unrestricted RN license, 5 years of clinical experience, and potential licensure in multiple states.

Qualifications

• Registered Nurse with current unrestricted license in state of residence.
• May be required to obtain other state licensure in states where Point32Health operates.
• National certification in Case Management desirable.
• 5 years’ relevant clinical experience required.
• Experience in home care or case management preferred.
• Proficiency in a second language desirable.
• Experience in specialty areas a plus.
• Bachelor’s degree or relevant equivalent experience required.

Responsibilities

• Perform telephonic member outreach and/or face-to-face encounters to enroll members and assess needs.
• Provide targeted health education and proactive strategies for condition management.
• Coordinate safe transitions to the next level of care and prevent readmission.
• Collaborate with the interdisciplinary care team to improve member outcomes and participate in rounds.
• Maintain professional growth through self-directed learning activities.
