Job summary

Somatus is seeking a dedicated RN Care Manager, VBC, to support high-needs CKD/ESKD patients. You will visit members in homes, dialysis clinics, and physician offices, collaborating with nephrologists and care teams to implement individualized care plans and improve outcomes.

Join a physician-practice partnership focusing on care coordination, education, and seamless handoffs. Travel within the designated region is required, with a strong emphasis on patient-centered, integrated care.

Qualifications

• RN license and ability to get licensed in other states as needed.
• 2+ years of RN experience in a multi-disciplinary team and with physicians.
• BLS certification from an approved facility.
• Renal, CKD or dialysis care experience as a focus.

Responsibilities

• Perform home visits and coordinate care with patients in clinics, homes, or facilities.
• Collaborate with nephrology practices to improve patient outcomes.
• Coordinate care plans with interdisciplinary team members and physicians.
• Educate patients on discharge requirements and self-management.
• Travel to member homes, facilities, and provider offices within assigned region.
