Job summary

Somatus is seeking an RN Care Manager, VBC, focused on high-needs CKD and ESKD populations. You will work with patients, nephrologists, PCPs, and care teams, performing visits in homes, dialysis clinics, and offices to support care plans.

This physician practice partnership role emphasizes collaboration, education, and navigation of barriers to care; travel is required within the assigned region, with telehealth and in-person visits enabling integrated, patient-centered care.

Qualifications

• RN license and ability to get licensed in other states as needed.
• 2+ years of RN experience in a multi-disciplinary team setting.
• Valid BLS certification from a licensed facility (AHA or Red Cross).
• Renal/Kidney disease or dialysis care experience as a main focus.
• Residence with high-speed internet or ability to access it.
• Travel to partner hospitals, clinics, and community facilities within the assigned region.

Responsibilities

• Outreach and scheduling face-to-face visits at homes, dialysis clinics, and physician offices.
• Build trust with physicians and practices to improve patient outcomes.
• Lead and participate in interdisciplinary care team meetings to coordinate care.
• Develop and implement individualized care plans with nephrologists and the care team.
• Educate patients and families about discharge plans, renal condition, and self-management.
• Coordinate care transitions and minimize readmissions.
• Provide guidance to non-licensed and licensed team members and delegate tasks.
