Job summary

Somatus is seeking a Registered Nurse Care Manager, VBC, to lead high-need CKD/ESKD patient care in partnership with nephrology practices. You will coordinate in-home visits, dialysis clinics, and office-based care, guiding the care team, and removing barriers to optimize outcomes.

You will travel to member homes and facilities, educate patients, and develop individualized care plans with the multidisciplinary team. This role emphasizes field-based collaboration and patient-centered care.

Qualifications

• RN license required and ability to get licensed in other states as needed.
• 2+ years RN experience, including multi-disciplinary team work with Physicians.
• Valid BLS certification from an AHA or Red Cross facility.
• Renal, CKD or Dialysis care experience preferred.
• Must reside where high-speed internet is available or have access to reliable internet.

Responsibilities

• Collaborate with physicians and practice staff to improve patient outcomes.
• Plan, coordinate, and lead regular interdisciplinary care team meetings.
• Establish and maintain relationships with patients and provider offices.
• Develop partnerships with provider teams to align clinical and operational goals.
• Educate provider practices on the Somatus program and workflows.
• Provide comprehensive care through in-person, telehealth, or phone follow-ups.
