Job summary

Somatus is seeking an RN Care Manager for the VBC program to support high-need CKD/ESKD patients. The role focuses on outreach, home visits, and coordination with nephrology practices to ensure timely, evidence-based care.

You will collaborate with physicians, PCPs, and the interdisciplinary team to implement individualized care plans and reduce readmissions while traveling within your region and using telehealth as needed.

Qualifications

• RN license; ability to obtain license in other states as needed.
• 2+ years RN experience in multidisciplinary care.
• Valid BLS certification from AHA/Red Cross facility.
• Renal CKD or dialysis care experience as main focus.
• Willingness to travel regionally to support care coordination.

Responsibilities

• Outreach and care coordination for high-need CKD/ESKD patients.
• Schedule home visits, dialysis clinics, and physician offices to support care plans.
• Collaborate with nephrology practices to improve patient outcomes.
• Educate and support patients and providers in care plan execution.
• Develop individualized care plans with the interdisciplinary team.
• Travel within assigned region to conduct visits 75% of the time.
