Job summary

Somatus is seeking an RN Care Manager, VBC to focus on high-need CKD/ESKD populations, coordinating care across physicians, practices, and patients. The role emphasizes in-person and telehealth engagements, care planning, and education to remove barriers to treatment.

You will travel within the assigned region to conduct home visits, support care transitions, and collaborate with nephrologists and the interdisciplinary team to optimize care outcomes.

Qualifications

• RN license in good standing with eligibility for required states.
• Minimum 2 years RN experience in multidisciplinary settings.
• Valid BLS certification from an approved provider.
• Renal, CKD, or dialysis care experience preferred.

Responsibilities

• Partner with physicians and staff to improve patient outcomes and build trust.
• Coordinate and facilitate regular interdisciplinary care team meetings.
• Establish positive relationships with patients and provider offices.
• Collaborate with practice teams to optimize workflows and care plans.
• Provide education and self-management support to patients.
• Travel to member homes, clinics, and hospitals to conduct visits.
