Job summary

Somatus seeks a RN Care Manager, VBC, to support high-need CKD/ESKD patients across our care network. You’ll partner with nephrology practices and PCPs, conduct in-home and clinic visits, and coordinate care plans. Travel within the assigned region is expected, with a focus on improving patient outcomes.

You’ll operate in a multidisciplinary environment, using MS Office tools, and participate in regular care management activities to deliver high-quality care and ensure patient safety.

Qualifications

• RN license current in Indiana or able to obtain in other states
• 2+ years RN experience including multi-disciplinary teamwork
• BLS certification from an approved provider
• Renal, CKD, or dialysis care experience preferred
• Must reside in area with reliable high-speed internet or ability to access it
• Willingness to travel to partner hospitals, clinics, and facilities within region

Responsibilities

• Conduct outreach, scheduling home visits, dialysis clinics, and physician offices for high-risk patients
• Coordinate regular interdisciplinary care team meetings with partner practices
• Develop and implement individualized care plans with nephrologists and PCPs
• Educate patients on discharge requirements and renal care
• Collaborate with providers to optimize workflows and improve patient outcomes
• Travel within the assigned region up to 75% of the time
