Job summary

Somatus is seeking a RN Care Manager to support high-needs CKD/ESKD patients. You will travel to member homes, clinics, and offices to drive care coordination and education in partnership with nephrology and primary care teams.

You will lead interdisciplinary discussions, develop individualized care plans, and monitor transitions of care while maintaining HIPAA-compliant communication with patients and providers.

Qualifications

• RN license required; eligibility to practice in target states.
• 2+ years RN experience, plus experience on a multi-disciplinary team.
• BLS certification from an approved provider.
• Renal/Kidney disease or dialysis care experience preferred.
• Must have reliable high-speed internet and be able to travel within the assigned region.

Responsibilities

• Coordinate care across home, dialysis clinics, and physician offices.
• Schedule and conduct visits with high-risk members; perform care planning.
• Engage with nephrology, PCP, and interdisciplinary teams to improve outcomes.
• Provide patient education on discharge and renal condition management.
• Lead and participate in regular care coordination meetings with partner practices.
