Job summary

Somatus is seeking an RN Care Manager (VBC) focused on CKD/ESKD populations to coordinate care across home, dialysis clinics, and physician offices. You will partner with nephrologists, PCPs, and care teams to implement individualized care plans, remove barriers to care, and improve outcomes for high-risk patients.

The role requires RN licensure, 2+ years of nursing experience, and ability to travel within the assigned region.

Qualifications

• RN license required.
• 2+ years RN experience in a multidisciplinary setting.
• BLS certification from a licensed training facility.
• Renal/Kidney/Dialysis care experience preferred.
• Must reside where high-speed internet is available and able to travel.
• Willingness to travel within assigned region for home, clinic, and facility visits.

Responsibilities

• Partner with physicians and practice staff to improve patient outcomes.
• Coordinate regular interdisciplinary care team meetings with practices.
• Establish relationships with patients and provider offices in-person and by phone.
• Collaborate with practice teams to optimize workflows and care plans.
• Educate practices on the Somatus program and integrated workflows.
• Deliver complete care coordination including home, telehealth, and clinic visits.
