Job summary

Somatus seeks an RN Care Manager, VBC to lead high-needs CKD/ESKD patients by coordinating care with physicians and practices. You will visit members in home, dialysis clinics, and offices, delivering education, assessments, and care planning with an emphasis on improving outcomes.

This physician practice partnership role requires strong collaboration, field-based travel, and the ability to develop individualized care plans with the care team.

Qualifications

• RN license required and ability to obtain licensure in other states as needed.
• 2+ years of RN experience, including multidisciplinary team involvement.
• BLS certification from a licensed facility (AHA/Red Cross).

Responsibilities

• Collaborate with physicians and practice staff to improve patient outcomes.
• Coordinate regular interdisciplinary care team meetings with partnered practices.
• Engage with patients and providers in-person and via telehealth to support care coordination.
• Travel to member homes, facilities, and clinics to conduct visits and care planning.
• Educate and guide non-licensed and licensed team members to support care plans.
