Job summary

Somatus is seeking an RN Care Manager (VBC) focused on high-needs CKD and ESKD populations. You will work with patients and nephrology practices to coordinate care across home visits, dialysis clinics, and offices, building trust and ensuring adherence to care plans.

This physician practice partnership role involves travel within the designated region to support care coordination, education, and improved outcomes, with onsite and telehealth interactions as needed.

Qualifications

• RN license in good standing; ability to obtain licenses in other states as needed.
• 2+ years RN experience, including multi-disciplinary teams and physicians.
• BLS certification from an approved provider (AHA or Red Cross).
• Renal, CKD or dialysis care experience as a focus of your job.

Responsibilities

• Partner with physicians and practices to build trusted relationships that improve outcomes.
• Plan, coordinate, and facilitate regular interdisciplinary care team meetings with partner practices.
• Establish and maintain patient relationships through in-person and telephonic engagement.
• Collaborate with provider teams to optimize workflows and care team processes.
• Educate providers on the program and promote integrated workflows within care plans.
• Provide complete care coordination across home visits, clinics, and facilities.
