Job summary

Somatus is seeking a RN Care Manager for VBC to support high-need CKD/ESKD populations. You will engage with physicians, practices, and patients to coordinate care, manage barriers, and educate on care plans.

Travel to homes, clinics, and offices is a core part of the role, with a focus on improving outcomes and reducing readmissions. Qualified candidates have an RN license, 2+ years of RN experience, and comfort with field-based care.

Qualifications

• RN license (and ability to get licensed in other states)
• 2+ years of RN experience, including multi-disciplinary team work
• Valid BLS certification from an approved facility
• Renal, CKD or dialysis care experience as a main focus
• Location with reliable internet or ability to access one
• Willingness to travel to partner hospitals, clinics and facilities in assigned region

Responsibilities

• Partner with physicians and practice staff to improve patient outcomes
• Plan, coordinate and facilitate regular interdisciplinary care team meetings
• Develop strong partnerships with provider practices to align clinical and operational goals
• Educate provider practices on the Somatus program and workflows
• Provide comprehensive care planning and education to patients
• Travel to member homes, clinics, and offices to conduct visits and coordinate care
• Oversee non-licensed and licensed team members in delivering care coordination
