Job summary

The role requires 2+ years in health care with care management experience, an active RN license, and strong communication skills. Some travel (25–40%) may be required depending on state contracts.

Qualifications

• At least 2 years of experience in health care, including at least 1 year in care management, and experience with persons with disabilities or LTSS.
• Active RN license in state of practice.
• Bachelor's degree may be required depending on state/contract.
• Valid driver's license and reliable transportation for travel.
• Strong verbal and written communication skills.
• Experience with substance use disorders in some states is a plus.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including in-person home visits as required.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members and care network to address needs and goals.
• Monitor care plans and document interventions and goal achievement.
• Promote integration of services including behavioral health and LTSS for continuity of care.
• Assess medical necessity and authorize waiver services.
• Facilitate ICT meetings for approvals/denials and collaboration.
• Educate and motivate members using motivational interviewing.
• Identify barriers to care and assist with psycho/social, financial, and medical obstacles.
• 25-40% local travel may be required.
