Qualifications

• At least 2 years of experience in health care including 1 year in care management or managed care.
• Experience working with persons with disabilities, chronic conditions, and LTSS preferred.
• RN license active and unrestricted in state of practice.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including in-person home visits.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members and care network to address needs and goals.
• Monitor and evaluate care plan effectiveness and document interventions.
• Promote service integration across behavioral health, LTSS, and community resources.
• Identify barriers to care and coordinate resources to address them.

Education

Bachelor's degree in health care related field
