Job summary

Qualifications

• Active RN license in state of practice.
• Experience in care management, managed care or LTSS settings.
• Ability to travel to member homes and care sites as required.
• Proven communication and collaboration with diverse teams.

Responsibilities

• Completes comprehensive member assessments within regulated timelines, including in-person home visits.
• Facilitates comprehensive waiver enrollment and disenrollment processes.
• Develops and implements care plans with members, caregivers and providers.
• Monitors care plan effectiveness and documents interventions and outcomes.
• Promotes integration of LTSS, behavioral health, and community resources.
• Assesses medical necessity and authorizes waiver services as appropriate.
• Supports ICT meetings for approval/denial of services and collaboration.
• Educates and motivates members using motivational interviewing and clinical guidelines.
• Identifies barriers to care and coordinates resources to address needs.
